FUTSAL TA

I hereby acknowledge and agree to the terms and conditions outlined in this waiver contract for my child's/children participation in the FTA Academy Program. By making the required payments and fulfilling the financial obligations, I am committing my child/children to the program for the duration of the period.

PAYMENTS

​

Fee Payments:

a. The fees for the FTA Academy Program may be paid in full or be in monthly installments, commencing from the day of the subscription creation and acceptance, following the payment of an initial $75 setup fee.

​

Method of Payment:

a. I understand that electronic payments via Stripe/Chargebee or bank transfer are the preferred methods of payment. Alternatively, checks are accepted as an alternative payment method.

​

Sibling Discount:

a. I acknowledge and agree that a sibling discount of 10% will be applicable to the fees of two or more children registered.

​

Refund Policy:

a. I acknowledge that FTA Academy (FTA) takes the commitment seriously. In the event that my child, who paid in advance, decides to withdraw from the program during the commitment period, FTA will retain 20% of the fees and refund the remaining balance.

​

Membership Pause/Cancellation:

a. I understand that should my child wish to be released from FTA during the membership period, I'll be charged a setup fee to reactivate/renew the membership.

COMMITMENT

​

Event Rosters:

a. I understand that each event roster may consist of players from and across all our program locations. My child may be given the opportunity to play up or across Academy programs, subject to the discretion of FTA.

​

Training & Events:

a. I acknowledge that my child is not required to attend all FTA training sessions and events, provided that prior notice is given. I understand that training groups are based on the player's birth year and/or futsal level.

MEMBERSHIP BONUS/FEATURES

​

Club Discount:

 

a. I understand that for any additional Futsal TA programs I wish to join outside of the Academy program, I am entitled to a 20% discount on the program fee.

 

Extra Events/Friendlies:

 

a. I understand that I will not have to pay for FTA's Winter or Summer festivals/tournaments, as they are included in the Academy plan.

 

Winter/Summer Camps:

 

a. I understand that to receive the included camp bonus as part of my membership, I must pay for at least 8 months of subscription within a 12-month period, or in advance, from the time I activate my subscription.

 

a.i. If I change my subscription, I will only be eligible for additional camps, beyond those provided in my previous membership, once I have completed the 8-month subscription payment within the 12-month period.

MEDIA RELEASE
I hereby grant FTA Holdings, LLC permission to use photographs, video recordings, or other media of my child captured during the Academy program for promotional or commercial purposes, without compensation and without further permission. I waive any right to inspect or approve the use of the images or recordings.

ACKNOWLEDGMENT OF RISK
I understand that participating in the FTA Academy involves risks that may include, but are not limited to, physical injury, psychological injury, economic loss, and death. I acknowledge that I have been made aware of these risks and accept full responsibility for any such outcomes that may arise. I agree to release and discharge FTA Holdings, LLC, its affiliates, and agents from all liability, claims, or causes of action that may arise from my child’s participation in the Academy.

By registering my child/children, I confirm that I have read and understood all the terms and conditions outlined in this waiver contract, and I agree to comply with them fully. I acknowledge that this contract applies for the FTA Academy Program and all fees and dues associated with it.
